Job Description

We're seeking a dedicated RN Case Manager to join our team in Milwaukee, WI.

What You'll Do:

As a Hospice RN Case Manager, you'll play a vital role in providing compassionate care to patients in their homes. You'll conduct assessments, collaborate with a care team to develop personalized care plans, and ensure patients receive the support and dignity they deserve. This is a unique opportunity to build meaningful relationships with patients and their families, offering them essential emotional and physical care.

Responsibilities:

Perform patient assessments and collaborate with the care team to develop and implement individualized plans of care.
Make referrals to other hospice care disciplines as needed, or document the rationale for not doing so.
Develop, prepare, and maintain accurate and timely patient care progress records according to care center policies.
Document pain and symptom management changes and outcomes as appropriate.
Teach patients, families, and caregivers appropriate patient care techniques.
Communicate regularly with the clinical manager and care team.
Perform on-call responsibilities and provide on-call services to patients and families as assigned.
Other duties as assigned.
What You'll Bring:

One year of nursing experience.
Current RN license in WI.
Current CPR certification.
Valid driver's license, reliable transportation, and liability insurance.
