Quality Improvement Manager
Monday – Friday Days, Weekends only if needed
DUTIES:
Knowledge & adherence to Federal, NYS laws/ regulations applicable to Home Care.
Participates in developing agency annual plan to improve quality of care and organizational performance.
Develops programs for staff development/ understanding of QI process & Develops MMIS for QI program.
Working in conjunction with clinical manager/CS, prepares reports for VP Clinical Services, & participates in PAC meetings.
Participates in Audits
Analyze performance data to identify areas for improvement.
Lead quality improvement projects and ensure adherence to best practices.
Conduct regular audits and inspections to ensure regulatory standards are met.
Prepare and submit required regulatory reports and documentation.
Oversee the development and implementation of clinical policies and procedures. Provide guidance and support to clinical staff on complex cases.
Develop and implement training programs for clinical staff.
Conduct orientation and ongoing education sessions to maintain high standards of care.
Mentor and support staff in their professional development.
Collaborate with healthcare providers to ensure coordinated and comprehensive patient care.Address patient concerns and complaints, ensuring timely and effective resolution.
Collect and analyze data on clinical performance and quality metrics.NYS licensed RN with minimum 3 years nursing experience Working knowledge of quality improvement and management information systems, HHA Exchange and Care Connect experience is helpful. Valid NYS Driver license ability to travel throughout the 5 Boroughs, LI and Westchester
